- Download the consent form
- Open it in Acrobat. If you need Acrobat, visit UGA's Adobe Creative Cloud page.
- Fill it out
- Rename the file to "
_consent.pdf" - e.g., "jsmith_consent.pdf"
- Attach it to this form
